Please note: These forms are only part of the IOLTA Grant
Application. Please refer to the Application Instructions for
details about other components.
- We are pleased to provide the Summary Form and Budget Form as
PDF files. The files include form fields that you may type directly
into while the form is open on your screen. To view and type on the
forms, you will need to download the free plug-in, Adobe Acrobat
Reader.
- Please review Proposal Summary Form
Help before completing the forms. It contains useful
information to ensure forms are filled in as accurately and
effectively as possible.
- The directions and forms have been changed from
the 2011/2012 IOLTA application. Please use the new templates.
